1. No Medical Services Provided Through This Website: This website is provided for informational and scheduling purposes only. No medical advice, diagnosis, treatment, or healthcare services are provided through this website. Medical services are provided only in person by licensed medical professionals after appropriate medical screening and consent.
2. No Protected Health Information (PHI): This website is not HIPAA-compliant. Users must not submit medical information including, but not limited to: Medical conditions, Medications, Allergies, Diagnoses, Symptoms, Pregnancy status, All medical information will be collected separately using a secure, HIPAA-compliant system prior to any treatment.
3. Appointment Requests: Submitting a booking request through this website does not guarantee an appointment. All requests are subject to availability, medical eligibility, and confirmation by Elysian Mobile Phlebotomy & IV Hydration.
4. Payment and Fees: Payment for services is due at or before the time of service unless otherwise agreed in writing. Prices listed on this website are subject to change without notice.
5. Cancellations and No-Shows: Clients are responsible for providing reasonable notice of cancellation. Late cancellations or no-shows may be subject to fees due to provider time, travel, and medical supply preparation.
6. No Guarantees: Elysian does not guarantee any specific results from IV therapy, injections, or phlebotomy services. Individual results vary.
7. Limitation of Liability: To the fullest extent permitted by law, Elysian Mobile Phlebotomy & IV Hydration is not liable for any damages arising from the use of this website or from appointment requests submitted through it.
